This study aimed to conduct a systematic review and meta-analysis of antimicrobial resistance patterns and treatment outcomes. morning In Southeast Asia, this zoonotic pathogen poses a major public health concern through the consumption of raw or undercooked pork.
A systematic review and meta-analysis was conducted by searching nine databases up to December 2024. morning Isolates or clinical outcomes of infected patients from Southeast Asian countries were eligible for inclusion. Pooled estimates were calculated with random-effects meta-analysis, and country-stratified analyzes were performed to examine temporal resistance dynamics.
Ten eligible studies from Thailand and Vietnam were identified, contributing 763 bacterial isolates and 618 patients. Pooled penicillin resistance was 2% (95% CI: 0-18%), with considerable heterogeneity observed across studies (I2 = 68.8%). Stratified analyzes by country revealed clear temporal patterns. Thailand exhibited nonlinear resistance dynamics, while Vietnam maintained consistently low resistance throughout the study period. Overall mortality in humans was 16% (95% CI: 11-22%). Ceftriaxone, penicillin, and vancomycin remained effective first-line treatment options.
Although penicillin resistance remains low overall, reports of moderate resistance require continued community surveillance and enhanced antibiotic stewardship in pig farming. Given the limited number of eligible studies and substantial heterogeneity, these results should be interpreted with caution.
