Lark Health, an artificial intelligence-powered chronic disease prevention and management platform, and Samsung on Wednesday announced a multi-year strategic partnership aimed at improving preventive care and health care for older adults, including Medicare beneficiaries.

Through this partnership, seniors will be able to access one or more of Lark’s 12 chronic disease management and prevention programs through Samsung Health at no out-of-pocket cost. Medicare-eligible beneficiaries will also receive care through live clinical interventions and health tracking devices connected to their mobile phones, the companies said.

Samsung has a footprint of 400 million devices, which will be combined with Lark’s AI health coaching capabilities.

Digital health cannot be measured by innovation alone, but “it must be measured by how digital health truly helps people (and at scale),” Hong Park, M.D., senior vice president and head of the digital health team at Samsung Electronics, said in a statement.

“Our Connected Care Everywhere initiative represents a seismic shift in how care must move away from fragmented, episodic care and toward a ubiquitous, always-on home health ecosystem,” said Pak. “Bringing Lark’s AI-powered health coaching into Samsung Health will give seniors everywhere access to clinically validated programs directly on the Galaxy devices they already own and use every day.”

Seniors will have access to the platform’s wellness program (called Lark Prevention), which offers AI coaching for stress, sleep, smoking cessation, and more.

Eligible seniors at risk for type 2 diabetes can enroll directly in Lark’s CDC-certified Medicare Diabetes Prevention Program through Samsung Health. The program includes a connected smart scale and personalized coaching at no additional cost to members.

For older adults with cardiometabolic and behavioral health conditions, including diabetes, Lark Associated Medical Groups will participate in CMS’ Effective and Scalable Solutions for Medicare Advancing Chronic Care (ACCESS) model through a program that combines clinical monitoring and AI-powered coaching through Lark Medical Group providers. Members will receive applicable connected devices, such as blood pressure cuffs and glucose meters, and care coordination will be shared electronically with the member’s primary care clinician, Lark Health said.

“Older adults face some of the toughest barriers to continuity of care, and this partnership directly addresses that reality,” Lark Health CEO and co-founder Julia Hu said in a statement. “By bringing Lark’s AI-powered 24/7 preventive and chronic care coaching and connected devices to Samsung’s impressive ecosystem and bridging it with innovative Medicare Access reimbursement, we are ensuring that every senior has access to always-on health support that can truly transform outcomes at no additional cost.”

According to the companies, these products will be available to Samsung users through Samsung Health from the third quarter of 2026 to early 2027.
