    Industry Voices — A new model for rural healthcare is taking shape

    For millions of Americans, where you live is quietly becoming one of the biggest predictors of how long you will live.

    In rural areas across the country, healthcare systems are undergoing rapid and irreversible changes. Since 2005, more than 200 local hospitals have closed, and many more are at risk of closure. The White House Rural Health Transformation Program, led by Dr. Mehmet Oz and the Centers for Medicare and Medicaid Services (CMS), has warned that the country is entering an era in which life expectancy is predicted by ZIP code.

    The health disparities faced by rural Americans are well documented. According to the Centers for Disease Control and Prevention (CDC), people living in rural counties are more likely than those living in urban areas to die prematurely from the nation’s five leading causes of death: heart disease, cancer, stroke, chronic respiratory disease, and unintentional injury. Many of these deaths are preventable with timely care. But access is a challenge.

    The traditional model of large hospitals acting as central centers of care is becoming difficult to maintain. In areas without hospitals, residents are forced to drive long distances for basic services and emergency treatment, or skip treatment altogether. They often add risk to people with critical health needs.

    This reality requires a different approach that combines technology, community partnerships, and a deep understanding of local needs. Community Health Plans is uniquely positioned to support that transition because our teams work collaboratively with local health care providers, pharmacies, and community organizations every day. This proximity is important because rural health care is not a one-size-fits-all system.

    Technology will play a key role in improving access. Advances in artificial intelligence and digital medicine are already increasing clinician efficiency. AI-enabled tools, when evidence-based, patient-centered, and used with proper clinical supervision, can help speed diagnosis and reduce administrative burden. Physicians are using technology to automate documentation and spend more time with patients. Advanced imaging systems instantly transmit scans to specialists hundreds of miles away, enabling faster decision-making and potentially life-saving treatment.

    The future of rural health care will not be driven solely by technology. It depends equally on strengthening the trusting relationships that already exist within the community. And few health professionals are as deeply integrated into rural life as pharmacists. Pharmacies remain one of the most accessible entry points to the healthcare system. Almost 90% of Americans live within five miles of a pharmacy. In many rural towns, a pharmacist may be the only readily available medical professional.

    Recognizing that reality, we have developed a long-standing partnership with the Pennsylvania Pharmacist Care Network through the EngageRx program. The program reimburses community pharmacists for previously unpaid services, providing an important source of revenue for businesses that are often under extreme financial strain. We encourage other plans to build on this vision and explore innovative strategies to meet the unique needs of the rural communities they serve, creating next-generation care plans that expand the role of pharmacists and enable them to provide care coordination, medication management, and support for health-related social needs such as food access and transportation.

    Care must be flexible and collaborative, especially in rural areas. Pharmacists, nurses, doctors, community health workers, and digital tools all have a role to play in helping patients navigate a system that can be difficult under the best of circumstances.

    The future of rural health care will be different than the past. It will be more decentralized, more collaborative, and more personalized to the needs of each community. Community health plans have a responsibility to work with government leaders, health care providers, and local organizations to help design that future and ensure that geography does not determine whether someone lives long and healthy lives.
