Suicide rates are disproportionately high among people living with severe mental health conditions, including non-affective psychosis. Non-affective psychosis refers to disorders such as schizophrenia, in which there is no predominant mood change like mania, and the main symptoms are related to a dissociation from reality. People with these symptoms often experience hallucinations, which are sensory experiences not shared by others.
For decades, psychiatric diagnosis focused on the physical severity of these visible symptoms. The intensity and frequency of hallucinations was thought to directly determine the patient’s overall health. However, clinical models are increasingly recognizing that the subjective emotional distress resulting from these symptoms has a significant impact on a person’s risk of self-harm.
Patricia Gooding, a psychology researcher at the University of Manchester, led a team investigating how emotional reactions to mental illness can lead to suicidal thoughts. Gooding collaborated with Camelia Harris, Sarah Peters, and Gillian Haddock. The group is affiliated with the University of Manchester and Greater Manchester Mental Health and NHS Foundation Trust.
The researchers designed this study to test a specific psychological theory about suicide. Many modern models of suicide are transdiagnostic, meaning they focus on psychological processes that occur across a variety of psychiatric conditions. These frameworks suggest that feelings of defeat and closure are the main psychological factors in suicidal ideation.
In the transdiagnostic model, the concepts of defeat and trap come from evolutionary psychology. Animals that lose a territorial fight experience a state of biological defeat and begin to give up and exhibit submissive behavior. If a defeated animal cannot escape from a dangerous situation, it becomes trapped.
In humans, this no-flight response is theorized to manifest as severe psychological distress. When people are completely caught up in their own mental suffering and feel that there is no possibility of salvation, thoughts of suicide may surface as a theoretical escape route.
Anxiety is another frequent struggle for people living with mental illness. Intense worry and panic can completely disrupt daily life and strain social relationships. Cognitive-behavioral models of psychosis posit that anxiety and psychotic symptoms interact with each other in a cyclical manner. Paranoia can cause severe anxiety, and high anxiety can severely worsen auditory hallucinations.
A team of researchers from the University of Manchester suspected that this cycle of anxiety and psychotic distress could eventually lead to deep psychological traps. People who feel trapped in their own minds may experience increased suicidal ideation intensity. They set out to map these specific psychological pathways using statistical modeling of clinical populations.
Researchers analyzed data from adult participants diagnosed with non-affective psychosis. These people were currently receiving care from mental health services in the UK. All study participants had experienced suicidal thoughts or behavior within the 3 months prior to observation.
The sample included 219 individuals, providing sufficient statistical power to test the intended psychological model. The demographics of this group were mostly male and primarily identified as white. The majority of participants were unemployed or formally exempted from employment due to health conditions.
Trained research assistants interviewed participants using a variety of validated clinical tools. One key assessment was a semi-structured interview designed to rank the severity of positive psychotic symptoms. Positive symptoms refer to the addition of clinical features such as delusions, rather than missing features such as flattened emotions.
To look beyond a simple diagnosis, researchers used a tool known as the Psychotic Symptom Rating Scale. This instrument breaks down the experience of voices and delusions into multiple dimensions. Interviewers measured the specific amount of psychological distress each person felt as a direct result of their hallucinations and delusions.
Participants completed several self-report questionnaires designed to capture their inner emotional landscape. They assessed their feelings of defeat, blockage, and hopelessness over the past few weeks. In this context, despair is defined as having very negative expectations about the future. The researchers also measured the intensity of each participant’s suicidal thoughts over the previous month.
The research team used a statistical technique known as mediation analysis to analyze the relationships between these different mental states. Mediation analysis helps scientists understand whether the relationship between a starting variable and an outcome is explained by intermediate steps along the way. The researchers controlled for common psychiatric symptoms to ensure that the observed associations were not simply a byproduct of a person’s widespread poor health.
Statistical modeling revealed strong indirect associations between psychological variables. Distress resulting from auditory hallucinations and delusions was, as expected, associated with the severity of suicidal ideation. This connection worked through channels that involved general anxiety and feelings of closure.
Specifically, people who experienced greater psychological distress due to their psychosis often experienced higher levels of baseline anxiety. This increased anxiety is coupled with a stronger sense of being trapped in our circumstances. Feeling trapped was the final link in the chain and was closely associated with very severe suicidal ideation.
The researchers tested this pathway backwards and found another reliable mathematical connection. Higher general anxiety levels predicted greater psychological distress in response to hallucinations and delusions. This distress was also associated with severe entrapment leading to severe suicidal ideation. The bidirectional nature of these findings supports the idea that anxiety and psychological distress operate in a mutually reinforcing feedback loop.
The research team also ran the same model replacing the statistical variable with psychological distress and using exact severity levels for hallucinations and delusions. When researchers looked only at the perceived intensity or frequency of delusions, none of the pathways leading to suicidal ideation were statistically significant. This suggests that simply experiencing hallucinations does not automatically lead to suicidal thoughts. The psychological response to the experience acts as the actual catalyst.
This finding regarding symptom severity and psychological distress represents a major conceptual shift for psychiatric research. Historically, reducing the physical severity of hallucinations has been the ultimate goal of drug treatments such as antipsychotics. This study suggests that a person may still hear voices frequently, but if the voices no longer bother them deeply, the risk of experiencing severe suicidal ideation may decrease.
Previous qualitative interviews with patients mapped neatly to these new statistical findings. People living with severe mental illness often describe an overwhelming sense of unrelenting. Continuing to struggle with mental health can create intense fear and a feeling of being completely overwhelmed by the illness.
Although this study provides a detailed psychological map of these internal processes, it does have limitations. This study utilized a cross-sectional design, meaning all interviews and questionnaires were completed at a single point in time. Because respondents were not followed continuously over long periods of time, researchers cannot say with certainty that one emotion specifically triggered another.
Another limitation involves how specifically the team measured anxiety. They utilized a single item from a broader clinical interview scale rather than a dedicated comprehensive anxiety questionnaire. There are many cognitive and physical aspects of anxiety that may not be captured by a single interview question.
The researchers advocate future studies using microlongitudinal tracking to address these remaining questions. This research method involves pinging participants multiple times a day with their mobile devices, recording their thoughts and feelings in the moment. Tracking moment-to-moment fluctuations in anxiety, distress, and feelings of closure can establish causal associations over hours or days.
This result presents an argument for prioritizing patients’ subjective emotional experiences in clinical settings. Modern treatments aimed at suicide prevention often focus broadly on mood and psychiatric diagnoses. The authors suggest that mental health professionals should work together to address the distressing feeling of being trapped in psychotic symptoms to potentially reduce the severity of suicidal ideation.
