    Millions of people suffering from joint pain and osteoarthritis lack access to the most powerful treatments.

    Stiff knees, lower back pain, and persistent joint pain are often dismissed as signs of normal aging. However, osteoarthritis, the most common joint disease worldwide, is not treated in accordance with actual research findings. Experts say the biggest gap is not a lack of options, but a lack of access to the most effective options.

    The most powerful treatments are not found in prescription bottles or operating rooms. It’s movement. However, in many countries and health systems, patients are rarely taught the only treatment proven to reduce pain and protect joints: exercise.

    Exercise is widely recognized as one of the most effective treatments for long-term joint diseases such as osteoarthritis. Still, only a minority of patients are prescribed it.

    Studies from Ireland, the UK, Norway and the US revealed a consistent pattern. Less than half of people diagnosed with osteoarthritis are referred to an exercise program or physical therapy by their doctor. More than 60% receive treatments not recommended by clinical guidelines, and approximately 40% are referred to a surgeon before nonsurgical options have been properly considered.

    Why exercise protects your joints

    To understand why this is important, it helps to look at how joints actually work. Osteoarthritis is the most common type of arthritis and already affects more than 595 million people worldwide.

    Key analyzes published in lancet It is estimated that the number of people living with osteoarthritis could approach 1 billion by 2050. Increasing lifespans, increasing sedentary habits, and rising rates of overweight and obesity are fueling this increase.

    Regular physical activity not only reduces symptoms; Biologically and physically reduces the risk of developing osteoarthritis and reduces its severity.

    Cartilage, the smooth tissue that cushions the ends of bones, has no blood supply of its own. Staying healthy depends on exercise.

    When you walk or put weight on a joint, the cartilage is slightly compressed, forcing fluid out. When the pressure is released, fluid is drawn in, carrying nutrients and natural lubricants with it. Each step helps nourish and maintain your joints.

    This is why describing osteoarthritis as simply “wear and tear” is misleading. Unlike tires, joints do not necessarily fail over time.

    Rather, osteoarthritis is better understood as a long process of breakdown and repair. Regular movement supports overall joint healing and plays a central role in maintaining good health.

    Osteoarthritis is a disease of the entire joint

    Osteoarthritis doesn’t just affect cartilage. This involves the entire joint, including the synovial fluid, underlying bones, ligaments, surrounding muscles, and even the nerves that control movement.

    Targeted exercises cover many of these components at once. Muscle weakness is one of the early warning signs of osteoarthritis, and strength training can help improve it. There is strong evidence that weaker muscles increase the risk of developing and rapidly progressing osteoarthritis.

    Neuromuscular exercise programs such as GLA:D® (The Good Life with Hip Osteoarthritis: Denmark) are designed specifically for people with hip and knee osteoarthritis. These programs are typically delivered in supervised group sessions led by a physical therapist and focus on improving quality of movement, balance, and strength. The goal is to increase joint stability and regain confidence in movement.

    Participants in these programs report significant pain relief, improved joint function, and improved quality of life that lasts up to 12 months after completing the program.

    Exercise benefits the entire body. Benefits have been proven across over 26 chronic conditions. With osteoarthritis, the effects go beyond just muscle strengthening and cartilage support. It also addresses inflammation, metabolic changes, and hormonal changes that contribute to disease.

    Inflammation, obesity, joint damage

    Obesity is a major risk factor for osteoarthritis, but it’s not just because of the increased stress on your joints. Excess body fat is associated with increased levels of inflammatory molecules in the bloodstream and joint tissues. These substances can damage cartilage and hasten the progression of the disease.

    Regular physical activity helps counter these effects at a molecular level. It can lower inflammatory markers, reduce cell damage, and even affect gene expression in ways that support joint health.

    Exercise before surgery

    Currently, there are no drugs that can change the fundamental course of osteoarthritis. While joint replacement surgery can be transformative for some people, it is a major surgery and does not work equally well for everyone.

    Exercise should be the first step and should remain part of care at every stage of the disease. It has far fewer risks than surgery and has far-reaching health benefits beyond the joint.

    Osteoarthritis is not just a disease that causes joints to “wear out.” It is shaped by muscle strength, inflammation, metabolism, and lifestyle factors.

    Regular, systematic exercise can help address many of these factors at the same time, protecting your cartilage, supporting your joints as a whole, and improving your overall health. Before resorting to surgery, exercise remains one of the most effective treatments available.
