A study led by Michigan Medicine and UT Southwestern Medical Center found that treatment with a “polypill,” which combines multiple drugs into a single daily dose, significantly enhanced heart function and reduced hospitalizations for heart failure patients.

Patients who took the polypill experienced greater improvements in left ventricular ejection fraction, a standard measure of the heart’s pumping ability, than those who received standard treatment with multiple drugs.

People taking the polypill also had 60% lower rates of heart failure-related hospitalizations and emergency room visits.

The survey results are natural medicine, We are proposing a new treatment approach that can help more patients with heart failure receive optimal treatment.

Polypill therapy has the potential to increase the availability of treatments that significantly improve outcomes in heart failure, but is still available to only a small proportion of patients. The promising results of the POLY-HF trial bring us closer to that reality. ”

Ambarish Pandey, MD, first author, cardiologist, University of Michigan Frankel Heart and Vascular Center, professor of internal medicine, director of the Center for Cardiometabolic Research, UM School of Medicine

Heart failure affects more than 6 million Americans. Despite advances in treatment, approximately 50% of patients die within 5 years of diagnosis.

Current guidelines recommend treatment of patients with heart failure with reduced ejection fraction (HFrEF) with four drug classes: beta-blockers, renin-angiotensin system inhibitors, sodium-glucose cotransporter 2 inhibitors, and mineralocorticoid receptor antagonists.

The combination of treatments reduces heart failure deaths by 50% compared to traditional dual-drug therapy, but only 15% of patients hospitalized with HFrEF receive all four drugs.

The study, conducted at UTSW from 2021 to 2025, randomly divided adults with left ventricular ejection fraction below 40% into two groups.

The first took a daily polypill containing three of the four drugs, and the second took a tablet of a renin-angiotensin system inhibitor. The other group received separate doses of each drug.

The researchers evaluated the patients after six months and tested their heart function. The polypill group had a greater improvement in ejection fraction, improving by 3.3 percentage points over the standard treatment group.

When researchers looked at whether patients were taking the drugs, nearly 80% of patients taking the polypill had all four drugs detected in their blood, compared to just over half of those taking separate pills.

While polypill therapy has been successfully tested in atherosclerosis trials, the POLY-HF trial is the first to test the approach in heart failure.

“POLY-HF shows that we can fill that gap by simplifying the way these drugs are administered, and this principle could be applied to many other conditions that rely on complex daily regimens,” said lead author Thomas J. Wang, MD, PhD, chair of the University of Michigan Medical School and Josiah Macy Jr. Professor, former chair of the Department of Internal Medicine at UT Southwestern.
